Job Summary
We are seeking a dedicated and compassionate Care Co-ordinator/Care Manager to oversee and coordinate care services within our home care settings. The ideal candidate will possess strong leadership skills, extensive experience in rostering and a thorough understanding of care. This role involves managing care plans, supervising staff, and ensuring the delivery of high-quality personalised care to our customers. The successful applicant will play a vital role in maintaining a safe, respectful, and supportive environment for all individuals under our care.
Duties
- Develop, implement, and review personalised care plans tailored to individual needs, including dementia and Alzheimer’s care.
- Coordinate rostering management to ensure consistency and quality of service delivery.
- Supervise and lead care staff, providing guidance, training, and support to maintain high standards of care.
- Oversee medication administration in accordance with regulatory requirements and best practices.
- Monitor customers’ health conditions, liaising with healthcare professionals as necessary.
- Ensure compliance with health and safety regulations within the care environment.
- Manage documentation accurately, including care records and incident reports.
- Foster positive relationships with customers’ families, providing updates on wellbeing and care progress.
- Lead initiatives for continuous improvement in service quality and customer satisfaction.
- You are required to travel to locations for first assessment, review of care, spot checks, recruitment, induction, managing on call pro rata and other day to day operations to provide exceptional quality of service.
Requirements
- Proven experience in home care environments.
- Strong background in nursing or senior care, with specific expertise in dementia and Alzheimer’s care.
- Knowledge of rostering are essential.
- Previous management or supervising experience within a care setting.
- Knowledge of developing and implementing personalised care plans.
- Excellent leadership skills with the ability to motivate and supervise staff effectively.
- Familiarity with medication administration protocols and regulatory standards.
- Compassionate approach with excellent communication skills.
- Ability to work collaboratively within multidisciplinary teams.
