Adult Social Care are currently looking to appoint a Social Care Assessor.

Do you have the right skills in delivering intensive support to individuals to enable them to provide the very best environments to Adults in the community to enable them to thrive?

We are seeking an Social Care Assessors to join our dedicated team. In this vital role, you will be responsible for ensuring the safety and well-being of individuals within our community.

The ideal candidate will possess a strong background in social work practices and a deep understanding of Adult Social Care policies and regulations. Your role will involve conducting thorough assessments, collaborating with multidisciplinary teams, and developing comprehensive intervention plans to support at-risk individuals. You will be tasked with not only identifying and addressing immediate concerns but also empowering people to achieve long-term stability and well-being. You will demonstrate exceptional communication skills, case management expertise, and the ability to navigate complex emotional scenarios with empathy and professionalism.

Excellent recording and report writing skills and IT are required for this role.

Skills

Essential:

  • Ability to carry out assessments, plan and deliver needs-based packages of support, balancing risk and protective factors.
  • Ability to build respectful, professional relationships with adults, families and carers, and to communicate effectively with people who may be experiencing distress, uncertainty or change.
  • Ability to gather information, listen carefully and contribute to assessments and reviews in a thoughtful and organised way.
  • Ability to work within clear guidance and delegated responsibilities, recognising when to seek advice, supervision or support.
  • Ability to identify concerns, risks or safeguarding issues and take appropriate action by following agreed procedures.

As this post meets the requirements of the Immigration Act 2016 (part 7), the ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post.

Knowledge

Essential:

  • Knowledge of the Care Act 2014, social care legislation, policy and procedures.
  • Knowledge of assessment frameworks.
  • A good understanding of working within adult social care.
  • Understanding of basic human needs, emotional social and physical.
  • Self-awareness of own strengths and limitations.

Experience & Qualifications

Essential:

  • NVQ level 3 or recent experience in an NHS Band 5 post (or equivalent).
  • Evidence of consistent pattern of learning from education, training and experience.
  • GCSE passes at Grade C or above or equivalent.
