Plan-It Homecare Ltd is a health care organisation providing supported living and domiciliary care services to people in the community. We work across 2 local authorities and looking to expand our reach and services. We are looking for an energetic and forward thinking individual who is willing to join our team to achieve the set goals.

We are now seeking an experienced, passionate and proactive Field Supervisor to join our growing team and play a key leadership role in the safe, effective and compliant delivery of our care services.

Key tasks for the ideal Candidate include:

1. Care Planning & Risk Management

  • Developing, reviewing and updating person-centered care plans and risk assessments for all clients
  • Ensuring care plans reflect each client’s current needs, preferences and goals
  • Conducting initial assessments for new clients before care begin

2. Staff Supervision & Support

  • Conducting regular one-to-one supervisions with care workers
  • Carrying out spot checks on care workers in the field to observe practice
  • Supporting staff appraisals and contributing to performance management
  • Mentoring and coaching new and existing staff

3. Quality Assurance & Compliance

  • Conducting audits of care records, medication administration records (MARs) and daily logs
  • Ensuring the organization meets CQC (Care Quality Commission) fundamental standards
  • Identifying gaps in care delivery and escalating concerns appropriately to the Registered Manager
  • Ensuring all policies and procedures are being followed correctly in the field
  • Completing incident and accident reports
  • Contributing to the organization’s continuous improvement

4. Safeguarding

  • Recognizing and reporting safeguarding concerns relating to adults at risk
  • Supporting or leading safeguarding investigations under the direction of the Registered Manager
  • Attending safeguarding meetings and contributing to multi-agency protection plans
  • Ensuring all staff understand and follow safeguarding procedures

5. Client Reviews & Welfare

  • Undertaking regular reviews of clients in their homes to assess whether care needs have changed
  • Responding to client or family concerns and complaints, working towards resolution
  • Building and maintaining positive relationships with clients, families and next of kin
  • Identifying any deterioration in a client’s health or wellbeing and liaising with appropriate professionals

6. Medication Management

  • Checking that medication is being administered correctly and records are accurate
  • Reporting medication errors and near-misses in line with policy
  • Ensuring Carers are competent to administer medication where required

7. Communication & Liaison

  • Acting as a point of contact between the office, care workers and clients
  • Liaising with GPs, district nurses, social workers, occupational therapists and other professionals
  • Attending multi-disciplinary team (MDT) meetings on behalf of the Organization
  • Handover of information between shifts and to the on-call team

8. On-Call & Emergency Cover

  • Responding to emergencies such as missed calls, carer sickness and client incidents from concerns escalated through the On Call
  • Covering care visits in emergencies where no other carer is available

Experience / Skills Required

  • Minimum 3 years’ demonstrable experience working in a domiciliary care setting in a senior staff position.
  • Proven experience working with clients who have age-related illness, learning disabilities and mental health conditions
  • Diploma Level 3 or above in Health and Social Care (or equivalent qualification).
  • Knowledge and understanding of safeguarding issues and the ability to address them appropriately.
  • Understanding of the role of social services, health, care, housing services and business development.
  • Excellent IT skills with the ability to maintain accurate digital records.
  • Excellent customer service skills with a professional and empathetic approach.
  • Excellent problem-solving skills with a proactive, solution-focused mindset.
  • Excellent communication and leadership skills, with confidence to liaise at all levels.
  • Positive attitude to change and a commitment to innovation and continuous improvement.
  • A genuine passion for making a meaningful difference in the lives of the people we support.
  • Full UK Driving License Required.

Schedule:

  • Monday – Friday: 4:00 pm – 8:00 pm
  • Saturday & Sunday: 7:00 am – 7:00 pm
  • Part-time or Full-time

Supplemental pay types:

  • Performance bonus

Education:

  • GCSE or equivalent (preferred)

Experience:

  • Management: 3 years (required)

Licence/Certification:

  • Full UK Driving Licence (required)
