Job Summary:
We are currently seeking a Rehabilitation Support Worker to provide structured support to a 26-year-old gentleman with an acquired brain injury and polytrauma. The focus of this role is on supporting routines – including meal preparation, promoting independence with daily living skills, and providing social and emotional support to wind down the day in a positive and safe way.
The client is a friendly young man with a great sense of humour. He enjoys listening to music, DJing, darts, Lego, and fishing. The support role involves helping him to plan and prepare meals, tidy up, and encourage structure and positive routines to promote wellbeing.
You will work closely with the client’s case manager and multidisciplinary therapy team to ensure consistent rehabilitation support.
The ideal candidate will possess experience in a care or support environment, demonstrating empathy, a genuine passion for providing support, and the ability to engage in friendly banter with clients. Excellent communication skills are paramount, and a full clean driving license is a prerequisite for all applicants. This role requires individuals with a high degree of confidence, initiative, and capabilities in lone working. Candidates should embody trustworthiness and a naturally caring disposition.
Key Responsibilities:
- Support the client with meal preparation, encouraging independence and healthy choices.
- Facilitate and promote engagement in activities, including light evening activities like watching a film or playing darts.
- Encourage and maintain daily living routines, supporting the client to complete household tasks where appropriate.
- Provide emotional and social support to help the client unwind after the day.
- Monitor and report any changes in mood, motivation, or physical wellbeing to the case management team.
- Maintain accurate and detailed shift notes at the end of each session.
- Liaise with the case manager and therapy team, following guidance and the rehabilitation plan.
- Advocate for the client when necessary.
- Uphold safety, dignity, and privacy at all times.
- Attend supervision and training sessions as required.
- Liaise with the case management team and report any concerns promptly.
- Transport to and from appointments and commitments.
- Light cleaning duties.
Qualifications and Requirements:
Essential
- Full clean driving licence and access to own vehicle.
- Clear DBS check.
- Minimum of 1 year of experience in a care or support role.
- Reliable, empathetic, and confident when lone working.
- Excellent written and verbal communication skills.
- Ability to engage positively with a young adult, using humour and motivation.
Desirable
- Experience or knowledge of acquired brain injury and/or polytrauma.
- Understanding of fatigue management and behaviour following brain injury.
- Flexibility to cover other shifts occasionally during staff absence.
