Description:
The Transition Director is responsible for coordinating all aspects of new admissions and individual moves as outlined in Gray policies and procedures. The ability to travel is required as transitions can occur in any area of the State. The Transition Director is responsible for the coordination of consumer referrals, admissions and moves. The Transition Director is responsible for ensuring that job duties are executed in keeping with Grays’ vision, mission and core values. The Transition Director explores and pursues growth opportunities and maintains accurate information related to job responsibilities.
– AGENCY GROWTH: Coordinate all aspects of the referral process and completely new admissions and consumer moves in compliance with Gray procedures. Present consumer information and analysis of the fiscal impact of potential new admissions to the admission committee prior to proceeding with the transition process. Ensure timely and accurate communication with internal and external customers during the transition process. Works with transition team to ensure transition checklists are accurate and up to date. Market existing services and programs to potential customers for all services. Schedule and attend provider fairs.
– Participate in Admissions Committee meetings.
– Complete all actions outlined on the Gray transition checklists. Tasks can include but are not limited to:
– Attend transition meetings
– Ensure completion of required documents such as Individual Profiles, Risk Protocols, Emergency Preparedness Plans, etc.
– Complete initial Individualized Training/BSP Training for new admissions
– Coordinate with the Human Resource Manager to set staffing schedules and hire for vacant positions
– Coordinate with the Gray Nurse to ensure medication and specialized training needs are met
– Coordinate with the Client Benefits Director to ensure that financial needs are communicated, and benefits applications are filed
– Coordinate and participate in moves including arranging moving teams, truck rental and new home set up[
– Maintain frequent contact with all involved parties throughout the transition process.
– Demonstrate/maintain professional internal and external relationships, cooperation and partnership with all Gray Residential employees, supervisors and individuals.
– Communicate issues promptly with the Program Director.
Minimum Qualifications:
· Knowledge
Current CPR Certification / First Aid Certification
High School Diploma or equivalent G.E.D
One-year direct experience working with individuals with developmental disabilities
Some supervisory/administrative experience is preferred
· Skills & Abilities
Ability to analyze and organize large amounts of information
Ability to write accurate and detailed protocols and plans
Ability to follow detailed plans accurately, including both oral and written instructions
Ability to apply sound, mature decisions
Ability to communicate adequately in order to complete required forms and reports
Ability to bend, stoop, sit, stand, reach and lift items weighing at least 75 pounds
· Additional Requirements
At least 18 years of age
Flexible schedule
Valid driver’s license, current liability insurance and dependable vehicle
Current negative TB (Tuberculin test)
Acceptable criminal (State and County) history background checks
· Relationships
This position works under the supervision of the COO. This position has direct contact with individuals receiving services, their families and/or legal representatives, case managers, behavior clinicians, other service providers, and community partners.
