We are a friendly, 3-site dispensing Practice in Buckinghamshire with surgeries in Wing, Whitchurch, and Winslow. The Practice has 6 GP Partners, together with salaried GPs and ANPs. They are supported by a full complement of Nurses, HCAs, a clinical pharmacist and provide care for 20,000 patients. We are a proactive and forward-thinking Practice dedicated to providing quality patient care. In total we are a team of more than 100 staff and we consider it important to maintain a supportive team ethos. We have a CQC rating of ‘good’ across all metrics, thanks to outstanding administrative support.

Job summary:
We have a vacancy for an experienced Prescribing Chronic Disease Practice Nurse on a part-time basis. Hours of work – 20 hours per week
Wednesday 14:00 – 18:00
Thursday 09:00 – 18:00
Friday: 09:00 – 18:00
Ideally you will have experience in a Primary Care setting, but training is available for the right candidate.
Working under the supervision of the Lead Nurse and in accordance with specific Practice Guidelines and Protocols, the Nurse will assist the Practice clinical team in the provision and delivery of prescribed programmes of patient care. Our Healthcare Assistants (HCAs) will play a pivotal role in conducting physical examinations or assessments that cannot be conducted virtually.
Duties and responsibilities:
- Focus on Diabetes Management
- Chronic disease management, including reviewing and agreeing care plans with patients living with asthma, diabetes, COPD and hypertension and accessing the effectiveness of these care plans on an ongoing basis.
- Assisting in the assessment and surveillance of patients health and well-being.
- Undertaking specific clinical activities for named patients that have been delegated and taught specifically in relation to that individual.
- Helping to raise awareness of health and well-being and how it can be promoted.
- Promoting health and well-being with our patients.

Knowledge and skills
Essential:
- Prescribing
- Previous management of patients with long-term conditions and complex needs
- Accountability of own role and other roles in a nurse-led service
- Local and national health policy
- Clinical governance issues in primary care
- Clinical leadership skills
- Communication skills, both written and verbal
- Capable or working autonomously and ability to build strong working relationships
- IT skills
- Up to date with training and yearly updates for chronic conditions including but not limited to Diabetes, Hypertension, COPD and Asthma.
Desirable:
- Working knowledge of EMIS Web and Docman 10
